If you are no longer the Lead OA please:

• forward the email to the lead OA in your unit
• send the name, telephone number and email address of the new Lead OA to the OAA Secretariat so that the records can be updated.

Method of Data Collection

We are using the OAA's survey system to collect the NOAD data for the year 2012. Instructions for submitting data will be sent with the email invitation.

It is suggested that you first print the questions to note the data prior to input.

DATA

Collection period: data is requested for the Calendar year (from January 2012).

Information from your unit may need to be collected from several sources, e.g., your lead midwife or lead obstetrician. Please complete the fields as accurately as possible. We have tried to define the fields tightly to avoid ambiguity and we recognise that some information is likely to be missing or not to be available retrospectively.

• Data does not have to be complete and you are asked to submit whatever you have

• If the answer to any of the questions is 'none' please enter '0'; if unknown, please leave blank
